Please read through the examples below before submitting:

• Sexual Harassment or Abuse – Inappropriate comments, contact, gestures, or behavior of a sexual nature.
• Bullying – Repeated, aggressive behavior that causes harm or distress (verbal, physical, social, or digital).
• Code of Conduct Violation – Disrespectful or unsafe behavior by athletes, staff, or parents.
• Inappropriate Staff or Coach Behavior – Yelling, favoritism, or unprofessional conduct—even if it’s not bullying or abuse.
